Remote Patient Monitoring

REDHS is excited to announce that we have a new service to offer the community – Remote Patient Monitoring.

The virtual care office (Regional Care Co-ordination Platform / Loddon Mallee) in partnership with the Better@home program has implemented a Remote Patient Monitoring (RPM) program utilising RCP as the platform focusing on complex care and chronic disease.

REDHS has been lucky enough to be included in this program alongside Heathcote Health, Inglewood District Health Service, Swan Hill District Health Service, Echuca Regional Health Service and Bendigo Health.

What is Remote Patient Monitoring?

Utilising integrated clinical devices the RPM program provides REDHS with the ability to monitor client conditions with the view to reduce preventable presentations and hospital admissions.

The virtual care office team have developed a patient portal which will be utilised by clients to engage with their clinical service, monitor their own results and track their progress.

Go Live dates are scheduled for the beginning of August with the view to expand the use of RPM into the future as an additional way we can deliver care.

Who is it for?

Anyone who :

  • Has a Smart Phone or tablet device
  • Has internet access
  • Has an email account on a smart device
  • Has at least one of the following conditions – diabetes, gestational diabetes, COPD, Heart Failure
  • Is likely to benefit from the program and self management support
  • Is able to complete the monitoring requirements
