Support at Home Program

Funded by the Commonwealth Government, Rochester and Elmore District Health Service (REDHS) Support at Home program provides local support and services to help older Australians live independently, maintain their health, and remain active in their community.

The program works in partnership with clients and their families to tailor supports that align with individual needs, preferences, and personal goals—supporting people to live safely and comfortably in their own home for as long as possible.

Eligibility

The Support at Home program supports older Australians who wish to remain living in their own home.

To be eligible, a person must:

  • Be 65 years or older, or 50 years or older for Aboriginal and Torres Strait Islander people
  • Meet Australian residency requirements
  • Live in the community (not in permanent residential aged care)
  • Be assessed and approved for services through My Aged Care based on their support needs and priority

Eligibility is determined through an aged care assessment.

Tailored Support at Home Services

Support at Home services are flexible and based on your assessed needs. Services are delivered in line with the Australian Government’s Support at Home service list and may include supports from the categories below.


Independence Supports

Support to help you live safely and independently at home, including:

  • Assistance with self‑care and activities of daily living
  • Assistance with communication and daily routines
  • Transport assistance to attend medical appointments and essential activities
  • Support with social connection and community participation

Everyday Living Supports

Assistance with everyday household tasks, including:

  • Domestic assistance such as essential light cleaning, laundry, ironing, and changing bed linen
  • Meal preparation and assistance with shopping

Home Maintenance and Garden Support

Basic services to maintain a safe and accessible home environment, including:

  • Essential light gardening such as lawn mowing and pruning
  • Minor home maintenance related to safety and access

(Services are limited to essential maintenance and subject to assessment and approval.)


Assistive Technology and Equipment

Access to supports that increase safety and independence at home, including:

  • Mobility aids
  • Bathing and toileting supports
  • Other approved equipment or minor home modifications

(Provided where prescribed or recommended following assessment, and subject to approval and availability.)


Clinical Supports

Clinical care provided by qualified health professionals, including:

  • Medication management
  • Wound care and dressings
  • Clinical continence support
  • Monitoring and support for ongoing or complex health conditions

Allied Health and Therapy Supports

Therapy services to maintain or improve physical, functional, and cognitive abilities, including:

  • Physiotherapy
  • Occupational Therapy
  • Podiatry
  • Exercise Physiology (individual or group programs)
  • Dietetics
  • Social Work

How to Get Started

An aged care assessment is the first step to accessing the Support at Home program.

To organise an assessment:

Assessments are conducted by a qualified aged care assessor through the My Aged Care assessment services | health.vic.gov.au either face‑to‑face or by phone.


What to Do While You’re Waiting

If you are waiting in the national priority system for Support at Home funding, you will usually receive a “get ready” letter around 90 days before funding is expected to become available.

This letter helps you:

  • Start planning for your care needs
  • Compare and shortlist approved Support at Home providers in your local area

Please note that this timeframe is an estimate and may change depending on priority level and funding availability.


Once you are approved for Support at Home services, you will be asked to choose a service provider.

Your chosen provider’s Care Partner will then contact you to work with you to develop a personalised support plan that reflects your needs and goals.

If you are waiting for your Support at Home funding to become available, interim services may be available, depending on your circumstances and eligibility.
