REDHS Services

Community Care Fees and Funding Options

Depending on your situation there are many ways to access REDHS Community and In Home Support Services, including Allied Health. If you are unsure which program may apply to you, or to find out further information please contact our Intake Officer on (03) 5484 4465.

My Aged Care – Commonwealth Home Support Programme (CHSP)

  • Are you over the age of 65 (or aged 50+ if Aboriginal or Torres Strait Islander?)
    • Do you need assistance with daily activities?
    • Have you been diagnosed with a medical condition or have reduced mobility?
    • Have you experienced changes in family care arrangements?
    • Have you had a recent fall, accident or hospital admission?

My Aged Care is a federal government initiative that allows you access to government-funded subsidies for aged care services.  The service will discuss your needs with you to identify what services will allow you to live independently at home as well as short term service options to help you become independent again.

Home Care Packages

Home Care packages are funded by the Commonwealth and provide a coordinated approach to care and services to enable people to remain independent for as long as is possible.

You may be able to fund the cost of services using your package funding. For more information, speak to a My Aged Care representative on 1800 200 422, or visit the My Aged Care website.

Home and Community Care Program for Younger People (HACC-PYP)

  • Are you under the age of 65?
    • Do you need assistance with daily activities?
    • Do you need support with health needs to become/remain independent?
  • Are you a carer of someone with a disability or chronic illness?

This HACC-PYP program allows access to subsidised services that provide care, support and maintenance to help people with disability, chronic illnesses or short-term health needs.

National Disability Insurance Scheme (NDIS)

The NDIS provides support to Australians who are permanently and significantly disabled to access therapeutic services and supports.

  • Are you aged between 7 and 65?
    • Do you live in and have Australian residency?
    • Do you usually need support from a person because of a permanent and significant disability?
    • Do you use special equipment because of a permanent and significant disability?
    • Do you need supports now to reduce your future needs?

Medicare Benefit Scheme: Chronic Disease GP Management Plan

The Medicare Benefit Scheme: Chronic Disease GP Management Plan scheme is delivered by your GP and supports those with chronic conditions or diseases to access allied health services. For more information, speak to your GP or visit the Department of Health website.

Department of Veterans Affairs

Services funded through DVA for Allied Health services require a referral from your GP (D0904 form). For more information, visit the DVA website.

Private Fee

If you are not eligible for the above funding options, you can still access our Allied Health services at Rochester and Elmore District Health Services as a private fee client. Please see the Community Care Fees – Fee for Service for more information.
